Financial Information

At Kathleen M. Kutalek, DDS, LLC, our doctor and staff are proud to be a team whose primary mission is to deliver the finest and most comprehensive periodontal services available today. We are concerned about your dental care and want to ensure you that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment policy.

FULL PAYMENT AT TIME OF SERVICE

• We require full payment at the time of service for initial consultation, emergency treatment and single-visit procedures. For additional periodontal therapy, we will first accept payment from your insurance company, then bill you for the difference.
• We also require full payment at the time of service if your insurance will only provide benefits to you.

INITIAL CONSULTATION/Emergency/Single-Visit Payments

For initial consultation, emergency treatment, and single-visit procedures, our policy is that payment is due in full at the time of service. For your convenience we accept cash, checks, money orders, and credit card payment (AMEX, Discover, MasterCard, and Visa).

Multiple Visit Payment Options

Option 1: Open Account Payment Plans – treatment fees divided into 2 or 3 payments

Prior to starting periodontal therapy, you may opt for an interest-free open account plan, which allows for 2 or 3 monthly payments. The first payment would be due at the time of treatment, and subsequent payment would be made during each month following treatment.

Option 2: Financing with Interest – for those who would prefer an extended payment plan

If preferred, an extended payment plan may be made beyond three months and up to 12 months duration with a service charge of 1.5% per month (18% per annum).
